| Documentation Required for Participant Membership |
In order for the Central Office to process your application, please complete and submit all necessary documents.
All Participants (other than Investigator):
- Roster Update form signed/submitted by the Lead CRA of the member network
- copy of Human Subject Protection Training certification
- CTEP ID included on the Roster Update form
Documents required for "Investigator":
- Roster Update form signed/submitted by the Lead CRA of the member network
- NCI # included on the Roster Update form
- copy of Human Subject Protection Training certification
- letter of support from the PI or RI of the institution
- current CV or NIH Biosketch
- signed Form 1572 Statement of Investigator and Financial Disclosure
